Lumen Christi Health Care Inc. is a home health care provider that offers both non-medical and medical (skilled nursing services and therapy) services. We work with a wide range of clients, from individuals who prefer to recover from an illness or injury in their own homes to elderly individuals who need assistance with the routine activities of daily life. Our caregivers are carefully pre-screened, and they’ll respect your privacy and your dignity.
When you work with us, we’ll perform a thorough evaluation of your needs and craft a care plan that’s tailored to your specific goals based upon that assessment. We implement programs and services to provide comprehensive quality home health care in the patient/client’s home. We involve you, your caregiver and designee, key professionals and other staff members in developing your individualized plan of care, treatment and services. Your plan is based upon identified problems, needs, goals, and physician orders. The plan is designed to increase your ability to care for yourself. All services are provided under the direction of the Administrator and the Director of Nursing. We address the patient/client’s unique psychological, safety, self-esteem, and self-actualization needs in a client plan of home health care. Some of our services include:

By providing health care education to clients and their caregivers, Lumen Christi Health Care Inc. serves as a resource for the client, the client’s family, and the community. Skilled nursing services will be provided by Registered Nurses and Licensed Practical Nurses (hired by Lumen Christi Health Care Inc.). Similarly, services provided by Physical, Occupational and Speech Therapists, medical Social Workers, and Home Health Aides will be provided to the extent that such professionals are either hired by Lumen Christi Health Care Inc. or contracted for by Lumen Christi Health Care Inc.
Our Agency maintains current and accurate personnel records, including verification of current license/certification, criminal background checks and health records.
Lumen Christi Health Care Inc. maintains open communication with clients, employees, and physicians via frequent contact in order to achieve quality and cost-effective care.
